You can use this Case Report form for player injuries as well as property damage claims.
Because it can be hard to get Cindy Pimental on the phone, you may have better luck asking questions by email. If you fax in your Case Report, it's a good idea to send Cindy an email asking her to confirm receipt and processing of your fax. CYSA-North forwards the Case Report to their administrator (Bollinger Insurance, www.bollingersoccer.com or www.bollingerinsurance.com) in New Jersey, who will pass it on to an Adjuster at the insurance carrier (Markel Insurance, in Virginia). You will be contacted by the Adjuster to confirm the details of the accident.
